The following is a brief summary of the project in which you are asked to participate. Please read this information before signing the statement below.
TITLE OF PROJECT: Stress, Well-Being and Resilience
PURPOSE OF STUDY/PROJECT: The purpose of this study is to explore the relationship between stressful life experiences and well-being, as well as the ways individuals cope with stress. This study has been reviewed and approved by Louisiana Tech University’s Institutional Review Board for Human Subjects.
You will be asked to complete an online survey about your life experiences and your thoughts and feelings about them requiring approximately 20-25 minutes.
PROCEDURE: To participate in this project, you must read and accept this consent form by checking the appropriate box below. Upon acceptance, you will be given directions to fill out a survey on www.surveymonkey.com. Participation will be voluntary and no identifying information will be collected in the survey (see IP address and “cookies” note below). Beginning the survey in no way obligates you to complete the survey. You may quit the study at any time with no consequences.
INSTRUMENTS: The survey includes questions about your life experiences, and domains including stressful life events, health, support, coping, and well-being.
QUALIFICATIONS: To participate in this study, you must be at least 18 years of age.
RISKS/ALTERNATIVE TREATMENTS: There are no foreseeable risks to individuals for participating in this study. However, if you feel uncomfortable at any time, you may withdraw from the study at any time. The participant understands that neither Louisiana Tech University nor the researchers are able to offer financial compensation or reparative medical treatment for participation in this study.
Please be aware of the following information regarding online survey research: the web server we use may collect data regarding your IP address and “cookies,” but it will not be used to identify you nor will it be viewed by anyone not on the research team. Once data collection is complete, we will delete this information.
BENEFITS/COMPENSATION: Although this study does not have direct benefits to the research participant, the knowledge developed may help others.
I attest that I have read and understood the description of the study, purposes and methods of the following study: “Stress, Well-Being, and Resilience.” I understand that my participation in this research is strictly voluntary and my participation or refusal to participate in this study will not affect my relationship with Louisiana Tech University in any way. Further, I understand that I may withdraw at any time or refuse to answer any questions without penalty. Upon completion of the study, I understand that the results will be freely available to me upon request. I understand that the results of my survey will be confidential, accessible only to the principal investigators, myself, or a legally appointed representative. I have not been requested to waive nor do I waive any of my rights related to participating in this study.
