This survey is for anyone in Somerset who has a neurological condition. It can be completed by yourself or your carer.

Neurological conditions result from damage to the brain, spinal column or nerves, caused by illness or injury. Neurological conditions include Multiple Sclerosis (MS), epilepsy, stroke, acquired brain injury, Parkinson’s, Motor Neurone Disease (MND), Huntington’s, myasthenia gravis, muscular dystrophy, and many other conditions.

The results of this survey will be published in a public report which will be shared with health and social care commissioners in Somerset.

This survey is being distributed by the Somerset Neurological Alliance (SNA). The Alliance is a group of patients and carers who are affected by neurological conditions.
