1. LETTER OF INFORMATION AND INFORMED CONSENT

This research study is being conducted by Natalie Vogt, a graduate student working with Dr. G. Fisk in the School of Policy Studies at Queen’s University in Kingston, Ontario, Canada.

This study will examine the effects of servant leadership on followers in a religious setting. You will be asked to respond to questions about the such topics as the extent to which you perceive your religious leader as embodying the characteristics of a servant leader, your level of trust in your religious leader, the extent to which you perceive yourself as being religious, as well as your level of engagement in your religious organization. It will take you about 15 minutes to complete.

There are no known risks associated with your participation in this study. Participation is completely voluntary. You are free to withdraw at any time for whatever reason by exiting the survey before the end. You are not obliged to answer any questions that you find objectionable. You will not be identified in any way if the results are published and nothing will connect you to your responses. All data will be stored in a secure computer file accessible only to the researchers for a period of three years, at which point the files will be permanently erased from the computer.
