WNCCHS is conducting an HIV survey about HIV care in western North Carolina. The survey is annonymous and will help us improve HIV care in western North Carolina. Thank you for participating in this short survey. This portion will allow you to give annonymous input about your current HIV care. It will help us highlight the good things happening in your care, but also identify areas where your care could be improved.

1. Thinking about HIV care, where do you receive your primary care? This would be your primary care doctor who you have regular appointments with and prescribes your medications, etc. If you answer "other" please specify who and where.

2. How often are your regularly scheduled appointments to see your doctor?

3. How often do you miss an appointment and NOT reschedule it?

4. In the past, if you missed an appointment that is scheduled what would be the main reason you missed it?

5. When you have a scheduled appointment do you receive a phone call to remind you of the appointment before the appointment date?

6. When you arrive for your appointment, how did you get there?

7. Off the top of your head, how long would you say your provider spends with you during your visit? This means the one on one time he/she spends with you.

8. When you finish your exam are you always scheduled at that time for your next exam? Either when you are checking out or by a nurse, etc.

9. Do you usually have to pay something when you check out?

10. Do you have any type of insurance that pays for or pays towards your care with this provider?

11. If you pay a Co-Pay when you check out, how much is that co-pay?

12. How do you pay for your medications?

13. How do you normally get your medicines?

14. When you pick up your medications are they usually ready to be picked up?

15. When you pick up your medications is a pharmacist available to speak with if you need it?

16. How would you rate the overall efficiency of the WNCCHS pharmacy?
