RCAN School Asthma Prevalence Survey 2009-2010 (Pre K- 6th grades)

The Regional Community Asthma Network of the Finger Lakes (RCAN) is a grant funded community asthma coalition that strives to improve asthma care of children. Partnering with Schools is essential to this task.

RCAN, semi-annually collects basic prevalence data to assist us with identifying trends for asthma in our area and specifically how we can better serve our Finger Lakes Region.

Please take the time to complete the following questions, to the best of your knowledge, for the 2009-2010 school year. A final report will be sent out via email by the Fall of 2010.
