Welcome to the Quality of Services Survey for Insight Psychological Centers and Inspire Health Centers. We are asking all individuals currently seeking treatment at Insight/Inspire to complete this survey, and invite those who have received treatment in the past to complete it as well. The purpose of this survey is to obtain information about your experiences at our treatment center so that we can improve, learn from your experiences, and continually work to offer the best possible services.

Please answer as honestly and openly as you feel comfortable, and indicate N/A if a question is not fitting or you prefer not to answer. Your answers questions on this survey are confidential and will only be reviewed by Insight's Directors of Compliance. Identifying information such as name, email, etc. is only being collected to ensure your receipt and completion of the survey.

In addition to completion of this survey, we also encourage you to openly voice comments and feedback about how your treatment is going to your treatment providers at Insight/Inspire. Your voice matters a great deal to us, therefore, in addition to this survey and feedback to your treatment team, you may also
express a concern by asking to file a formal grievance report with our front desk Receptionist. All formal grievance reports will be managed and reviewed by the senior leadership team in conjunction with the reporting individual.
