The Ready by 21® Partnership is pleased that you have decided to apply for its recently announced Quality Improvement & Asset Building Challenge. This survey is intended to serve as your community's application to participate in the Challenge. Please complete the survey form no later than 5 pm Eastern Time on June 18th.
