Survey of Public Dignity Champions

 

The Dignity in Care Campaign wants to encourage more members of the public to join the campaign as Dignity Champions. It also wants to provide better support to public Dignity Champions and give them the tools and resources they need to be able to make a difference.

We would be grateful if you could complete this short questionnaire to help us answer some key questions to be able to do this. Your responses will be anonymised – we do ask for some personal details but please feel free to omit this section if you wish.
