Thank you for participating in Habitot's 8th Annual "Preschool Teachers Make a Difference" Awards! Please see the first page of this survey for ELIGIBILITY and SELECTION PROCESS.
All nominated teachers win an award and ten teachers are selected as honorees based on the nomination statement.

Our ten honorees will receive the following AWARDS:
• $100 worth of school supply gift certificates
• A Folkmanis puppet
• A "Chinook Book" coupon book
• A FREE preschool field trip to Habitot (up to 15 students ~a $175 value)
• A Habitot 5-Visit Pass (up to 3 people per visit; an $99 value) which may be offered at a fundraising auction or used personally • Plus four free tickets to the award ceremony celebration with guest speakers, musical entertainment, appetizers, a gift auction, and more.

ABOUT THE HABITOT AND THE AWARDS:
For eighteen years Habitot Children's Museum, a nonprofit organization based in Berkeley CA, has served over 1,080,000 families with family programs, immersive play-based exhibits, outreach and parenting education. Habitot believes that all children should have the opportunity to live with joy and learn though play, regardless of their income, life history, or physical ability. Habitot offers rotating exhibits, such as a Medical and Veterinary exhibit for roleplaying in the doctor's office, a child-sized kitchen cafe and grocery story, a water play zone, wind tunnel, infant toddler garden, and an art studio with unlimited art supplies and a personal art teacher to demonstrate techniques and offer guidance for families, caregivers, and tots. Habitot offers special programs, at no cost, to families with children experiencing special needs, children experiencing homelessness, children in the foster care, children with formerly incarcerated parents, children with teen parents, LGBTQ families, children speaking English as a Second Language and more.
