Essential Components of Care Coordination Chart

 

On the next pages are the components for Care Coordination. You should have received in the email the chart with the components and some recommendations that have already been completed.
The purpose of this survey is to get any more comments/recommendations on the components from the design team. We will then compile all of the recommendations and present to the group at the next meeting.
