About the transport service you used
This survey is for patients who are registered with a Somerset GP and who have used transport to get to and from a hospital or health clinic appointment in the last year that was not their own car; for example, where an ambulance or a hospital car was booked so that a patient could attend an appointment.
The purpose of this survey is to find out whether patients understand how hospital transport works, whether staff need to be better trained in explaining this to patients, or whether the transport arrangements can be improved.
