Quality of Life Survey for Parental Caregivers
1. Introduction
Welcome to the Survey!
The purpose of this survey is to investigate possible factors that may promote successful adaptation in parental caregivers who are providing care to a co-residential child with a developmental disability. If you are older than 18 and are a parent who provides care to a co-residential child (either a minor or an adult) who has been diagnosed with a developmental disability, you are eligible to participate in this study.
This survey will take about 20-25 minutes to complete. Your participation in this study is completely voluntary and anonymous. You may withdraw from the study at any time with no penalty. The survey information will not include any identifying information (e.g., names, addresses, personal information) to help ensure your anonymity.
In appreciation for your time and effort, you can opt to enter a drawing for one of 30 $50 Target gift certificates upon the completion of the survey. This study is inviting an estimated 435 participants; therefore the estimated odds of winning a gift card is one out of 15. If you would like to enter the drawing at the end of the survey, you will be asked to click a hyperlink and then to enter your email address. The system will then notify the author that a respondent has been entered into the drawing. Efforts will be made to keep your personal information confidential. The investigator will only view the email addresses of those who win the gift cards. Your email will be stored separately from your survey response and will be destroyed after the drawing is completed. This procedure is used to ensure that your name and email address will not be collected. Moreover, the investigator will not be able to track back to your response through your email address.
