1) This information is essential to our ability to understand your special skills, expertise, and services and will provide valuable information to the many hundreds of families and individuals that call the National Center each year looking for assistance and support.

2) This form is intended for individual consultants only, and if you are part of a larger firm, all individual consultants within your firm should fill out this questionnaire in order to be considered for future referrals.

3) Finally, please note that the National Center is not responsible for the actions or recommendations of the consultants it refers, and that we receive no direct compensation or payment for this referral.

PART V. FEE SCHEDULE: Please provide some sense of your fees. If you are uncomfortable listing a specific rate, at a minimum provide a range of fees you have charged family foundation clients over the past two years. Again, failure to answer these questions will make it difficult for us to refer your services to families that contact us for referrals.
