The Wisconsin Department of Health Services (DHS) made changes that have affected some people’s Medicaid personal care service levels (NOT supportive home care services) and is proposing changes for personal care travel to and from a person’s home. The Survival Coalition is collecting information to understand how these changes might impact the lives of individuals and their families. Please take a few minutes to complete the following survey.
