Dear Interested Participant,

My name is Elizabeth White Chaisson and I am a graduate student at the University of Arkansas training to be a therapist. I am a native of Auburn and graduated from both Auburn High School and Auburn University. I also worked at Lee OB/GYN for several years during my undergraduate years, which led me to become very interested in the relationships between life stressors and physical health.

I would like to ask you to consider helping me with a study that will help us better understand how life stresses can affect your physical health. Your feedback on your experiences would help us learn how to better understand how life stresses affect women’s health.

We know that major negative life experiences put you at risk for long-term health problems like, heart disease, diabetes, arthritis, and chronic pain disorders. Women are much more at risk for these and other health problems, because they are at greater risk of experiencing violence. For these reasons, each of the doctors at Lee OB/GYN has been very interested in learning more about how negative life experiences affect the overall health of their female patients.

If you answer this survey, you will have a chance of winning a $100 gift certificate to everyone’s most visited store, Wal-Mart! You must be at least 18 to participate. All you have to do is continue on the website, and answer questions regarding your life history, stress, sources of strength, and physical health. The doctors of Lee OB/GYN have allowed me to access your files to review your visits in the past year. AS SOON AS THE MEDICAL INFORMATION AND THE QUESTIONNAIRE RESPONSES ARE COMBINED, YOUR IDENTIFYING INFORMATION WILL BE REMOVED FROM THE DATA AND A RANDOM NUMBER WILL BE ASSIGNED, MAKING YOUR ANSWERS AND MEDICAL RECORDS COMPLETELY ANONYMOUS. All your answers and information will be kept completely confidential to the fullest extent of the law and University policy. General information about group results will be shared with physicians at Lee OB/GYN to help them meet the needs of and serve their patients more effectively.
