NHS Kingston Community Pharmacy Services Questionnaire

1. Community Pharmacy and You

Thank you for agreeing to take part in this questionnaire.

The results of this survey will be used to support the development of a Pharmaceutical Needs Assessment in NHS Kingston. This assessment is designed to review the current status of pharmacy services in the NHS Kingston area, examine gaps in provision and where necessary introduce new services to support the needs of local populations.

It likely that a final draft report will be available in late September 2010 for consultation. We would encourage you to visit the trust website at www.kingstonpct.nhs.uk for further information around this time to make your comments on the draft report.

Please answer these questions as honestly as you can. No personally identifiable data will be requested so you can be assured that anything you say will be anonymous.
