Thank you for taking time to complete this survey. Information obtained from this survey will be used to better address the needs of LGBT persons in Northeast Florida. You do not have to be LGBT to participate. Participation in the survey is voluntary and anonymous. Responses will be grouped and no attempt will be made to connect identifying information to any individual. It takes approximately 10 minutes to complete the survey. Please try and answer every question as accurately as possible, but, if you reach a question you feel uncomfortable answering, you may skip it.
