CDH Student iPad Agreement Form

 
  • One form must be filled out for each student.
  • Please return this form ASAP, no later than August 22, 2012.
  • iPads will not be distributed to students who do not have a completed form on file, nor will students be allowed to use their own iPads in school without having completed a form.
  • You can click the link in your email as many times as needed to submit the agreement for all of your students.

4. Student ID Number (optional, but helpful): Student ID numbers can be found through the Infinite Campus portal, or by checking a student's schedule.
An Apple ID (iTunes account) is required for each student in order to set up their iPad and download apps (including CDH required apps). The ID is required on this form so that we can assist students in setting up their iPads. If you do not already have an Apple ID, you have the option to set it up with or without a credit card. Instructions on setting up an account with no credit card can be found here.
