Please help us to evaluate the services provided by the Insurance Tax Section by completing the 12-question survey provided on the following page. The Insurance Tax Section processes all premium tax reports and payments. Your feedback will help us focus our efforts so we can provide the best service. The survey generally takes less than 10 minutes to complete.
YOU MAY KEEP YOUR SURVEY RESPONSE ANONYMOUS. You do not need to include your name or any other identifying information in the survey response. Item 11 asks your company type. We use this information to determine whether customer satisfaction is consistent among various types of companies.
