INTRODUCTION

Thank you for your interest in this study that is investigating how gene-based diet approaches and digital reality technology environments may affect health behaviours. If you are between 19 to 26 years of age, we ask that you complete this consent form first. If you consent, we will send you a screening form that helps us to know your eligibility to participate. If you are eligible, then we will ask you to complete a second online form that collects some background information about you. This will take about 20 minutes to complete. Also, you will be sent a package that contains food records to complete over 3 days. Both the online questionnaire and items in the package will need to be completed before the first in-person visit at Kwantlen Polytechnic University, Surrey campus.

This consent form will give you the basic idea of what the research is about and what your participation would involve, if eligible. If you would like more detail about something mentioned here, please email the Researcher at: Karen.davison@kpu.ca.

Take the time to read this carefully. By providing consent through this online form, you are granting permission for us to: 

1) collect data about your eligibility; 

2) collect information about your health (if eligible); and 

3) collect information about your food intake (if eligible). 

If you proceed with the study, you will review a paper version of the consent form at your first in-person visit to sign and confirm that you agree to continue to participate in the study.

At some point in the study, you will be involved with educational interventions involving digital reality technologies (virtual and augmented reality). 

PURPOSE AND STEPS OF THE STUDY

The purpose of this study is to investigate how different approaches to gene-based dietary education may lead to changes in health behaviour. Study participants will receive a DNA (gene) test, known as a nutrigenomics test, which assesses your body's ability to respond to dietary factors (e.g., carbohydrate, fats, sodium, vitamins). The test results will be accompanied by diet information for you to follow. If you are eligible, you will go through five steps in this study.
