The MGH Center for Community Health Improvement & Spaulding Rehabilitation Hospital are partnering with residents, local agencies and organization and we are pleased to release Charlestown’s 2015 Quality of Life Survey. A version of this survey was given in 2012 and resulted in Charlestown working on new and existing priorities in different ways and having new conversations focused on collaboration and change.

This survey, in combination with conversations in the community and updated data from state and local sources, will be used to help identify health needs and collectively determine ways to address them. We plan to repeat this survey every three years to gauge our progress in improving the quality of life in Charlestown.

We want to hear from you!

Please take the next 5-10 minutes to answer the attached questions. By answering these questions you will be able to share what it is like to live or work in Charlestown and what issues you think are important for the community to work on. Your responses are completely anonymous. The survey will not ask for your name. If you do not feel comfortable answering a question, you may skip it.

Charlestown is a diverse and vibrant community rich with history and wonderful traditions. Like any community, Charlestown is faced with many issues that affect health, such as poverty, drug addiction, violence and pollution. Collectively we can work to make Charlestown an even better place, but we need to hear from you.
