1. General Questions

 
This questionnaire is designed to gather information in order to help provide remedies to the barriers that persons encounter in their application for, and receipt of, public assistance and other benefits. The benefits we are concerned about include cash assistance, food stamp benefits, emergency assistance, and domestic violence assistance. We are also interested about the fair hearing assistance. The goal of the Hunger Action Network of New York State’s project is to identify those local social services districts that have policies and practices that create barriers for people who are trying to receive assistance under any of the various benefit programs administered by the districts. We want to identify those barriers and perhaps learn of some new ones not considered in this survey.
