This questionnaire is designed to capture information on triggers used for hematopoietic progenitor cell collection by apheresis (HPC(A)). To fill out this survey, you will need specific information about your program’s activity during the 2011 calendar year, including the laboratory triggers for HPC collection, the number of HPC collections performed, and the standard target number of HPCs required at your institution for different indications of autologous stem cell transplantation. If these data are not available for 2011, please use the 2010 calendar year. Please collect this information before filling out the survey. Your institutional identifying information will be kept confidential. Data gathered through this survey will be used to understand current clinical practice. We request that each program designate one person to respond to this survey and limit the number of respondents from any individual program to one person. This questionnaire surveys information on the nature of your institution, HPC (A) collection activity, collection triggers and mobilization regimens. Estimated time to complete survey: 15 minutes
