填表須知:
1. 如對本局有任何投訴，請填妥以下表格。本局將會謹慎處理所有投訴，達以改善本局之服務。
2. 本局可能要求投訴人提供更多資料，以協助本局處理有關投訴。
3. 如投訴人選擇以匿名作出投訴，本局將無法向投訴人收集更多資料，從而令投訴無法得到適當的處理。
4. 投訴人將獲秘書處回覆其投訴已收悉。
Notes for Attention:
1. Please use this form to lodge a complaint about any aspect of the HKCAAVQ. We take complaints seriously and regard them as important in helping us improve our services and practices.
2. The complainant may be asked to provide additional information to help us process the complaint.
3. If the complaint is made anonymously, the HKCAAVQ will not be able to collect additional information for investigation and further processing.
4. All complaints received will be acknowledged.
