We would like to ask you a few questions about your health, nutrition, and community involvement in order to learn more about the 11th grade integrated curriculum unit. Your answers are completely confidential, meaning this is a completely private survey. No names are attached to this survey. Instead, you will create a secret code that only you will know.

You may skip any questions that you do not want to answer and can stop taking the survey at any time. We greatly appreciate your taking the time to answer these questions for us!

The first four questions are to create your own secret code. When you take the survey again at the end of the school year we will ask you the same four questions. This way we can match your answers from both the fall and the spring, but we'll never know it is you, because no names are ever attached to the surveys.
