My name is Jason Rhoades, and I am a doctoral student in the Public Service Leadership Program at Capella University. My doctoral advisor is Dr. Leech. My research is entitled A QUALITATIVE INVESTIGATION OF DISABILITY POLICY AND PRACTICE IN A NORTHWESTERN PENNSYLVANIA COMMUNITY. The purpose of the study is to identify individuals with disabilities and gain knowledge about their perceptions on the effectiveness of disability policy. You are invited to participate in this research because you are 18 years of age or older, and you have a disability.

Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in this research survey, you may withdraw at any time. If you decide not to participate in this study or if you withdraw from participating at any time, you will not be penalized.

The procedure involves completing an online survey that will take approximately 30 minutes. Your responses will be confidential and no identifying information, such as your name, email address or IP address will be collected. The survey questions will be about your perceptions of disability policy. You will be notified if any new information is revealed that may have bearing on your decision to participate in this study. Your identity will not be disclosed to anyone including the researcher, as SurveyMonkey allows for complete protection of your anonymity. All information from this study will be stored on a removable storage device, and stored in a safe when the information is not being used for research.

SurveyMonkey will do their best to keep your information confidential. All data is stored in a password protected electronic format. To help protect your confidentiality, the surveys will not contain information that will personally identify you. The results of this study will be used for scholarly purposes only and may be shared with Capella University representatives.
