Family Member/Significant Other of Person with TBI Needs Assessment Survey

 

Thank you for taking the time to complete this survey of family members and significant others of people with traumatic brain injuries (TBI). We are using this survey to find out what are the needs of both people with TBI, and of their family members. Using this information, we can come up with a plan for meeting those needs with public and private resources.

It would help us the most if you answer all the questions, but all questions are optional. If you don't want to answer a question, just skip it.
