If you are completing this survey online, you have accessed a link inviting participants to take part in research regarding their extraordinary experiences. Only participants over the age of 18 are able to take part in this study. This research is conducted by Dr Janette Simmonds, who is a senior lecturer and psychologist at Monash University. The aim of the research is to investigate extraordinary or exceptional experiences and effects they may have in contributing to psychological growth and change. A better understanding of which personal experiences have an impact and in what circumstances may be helpful in facilitating opportunities for promoting constructive personal experiences, and promoting and sustaining the beneficial effects of the experiences to optimise their benefits for mental health.

The survey takes approximately twenty five to thirty minutes to complete. It is possible that some survey questions may cause you some discomfort. You can choose whether or not you respond to particular survey questions. Participating in this study is voluntary and you are under no obligation to consent to participate. However, if you do consent to participate, you may only withdraw prior to submitting the completed questionnaire online as it will not be possible to determine which questionnaire belongs to you. All of the information provided from the surveywill not, under any circumstances, contain names or identifying characteristics. Any information you provide is confidential and no information that could lead to the identification of any individual will be disclosed in any reports on the project, or to any other party. Data from the survey will be kept in secure storage and accessible to the research team at Monash University. This data will be destroyed after a 5 year period.
