Everyone Counts NHS Survey for Hampshire, Isle of Wight, Southampton and Portsmouth

 

Hello.

Your local NHS would like to hear about your experiences of using health service across Southampton, Hampshire, the Isle of Wight and Portsmouth.

The local population is very diverse and we are determined to ensure that the services provided by local NHS organisations are of the highest quality and meet the needs of all local residents. We also recognise that some people face additional difficulties or barriers when trying to use health services or improve their health.

All the NHS organisations in the area have worked together to design this survey. Your answers and comments will help us understand what is working well and where we need to concentrate on making improvements.

There are questions about:

- Using health services in the community, like seeing a GP or dentist
- Questions if you have recently stayed in hospital - this could have been for treatment of a physical condition or a mental health problem
- Specific questions for people who use mental health services
- Questions to find out a little bit about you

We will not ask for your name, and everything you tell us is confidential.

Please answer all questions that apply to you. If you would prefer not to answer a particular question, just miss it out, and move on to the next one.

It will take about 10 minutes to complete the survey. It may take a little longer if you want to give us more detailed information about your personal experiences.
