The National Council for Palliative Care (NCPC) is seeking to work with The Princess Royal Trust for Carers and Crossroads Care to explore the needs of carers supporting someone with a life-limiting condition and those approaching the end of life. NCPC is an umbrella charity for adult palliative, hospice and end of life care.

We hope to gather information that will assist production of guidance and tools to help carers’ organisations provide support for families providing care for somebody at the end of life. We would like to hear your experiences and would be grateful if you could answer some questions below.

* 1. Do you come into contact with carers of people who have life-limiting conditions or are close to the end of life? "Close to the end of life" is taken to mean the last 12 months of life.

* 2. In your experience, are plans and choices for the end of life discussed with the cared-for person and the carer(s)?

"Plans for the end of life" can include a broad range of conversations and actions, from initially broaching the subject with someone who is relatively well, to making practical arrangements such as where the person wants to be cared for and eventually die, or whether there are certain interventions they would not want, to writing a will, planning a funeral, and discussing bereavement.
