This survey is for people who enquired about becoming egg, sperm or embryo donors in England, Scotland, Wales and Northern Ireland, but either were not able to go ahead or chose not to.

We would find it very helpful to know the reasons why you chose not to or were not able to become a donor. We would also like to be sure that you were treated with respect throughout the process.

The NGDT will use the information you give us to work with clinics to improve the experience for future enquirers, whether they are able to become donors or not. It will also be used for research papers on improving donor care. You will never be personally identified in any report or publication based on this survey.
