DIABETES HEALTH AND WELL-BEING SURVEY

Nicola Gawlik is an Honours student in the School of Psychology at Flinders University, South Australia. She is undertaking research leading to the production of a thesis concerning the health and well-being of FEMALES (only females!) with Type 1 diabetes. Participants need to be at least 18 years of age.

She would be most grateful if you would volunteer some time to assist in this project by completing this questionnaire that addresses certain aspects of this topic. Information about your quality of life is sought, as well as lifestyle issues associated with diabetes, your attitudes toward the future, and general information about your diabetes management. No more than about 30 minutes of your time is required.

Be assured that any information provided will be treated in the strictest confidence and no participant will be individually identifiable in the resulting thesis, report or other publications. You are, of course, entirely free to discontinue your participation at any time or to decline to answer particular questions.
