CyberQUEST Teacher Survey 1

Dr. Kathy Hayden and Dr. Youwen Ouyang, faculty researchers at California State University San Marcos, are conducting a project called CyberQUEST funded by the National Science Foundation. As participants in this study you are asked to complete the following survey during the teacher academy and at the end of each year.

This survey asks about your own technology experiences and the environment in which you teach. Your answers will help CyberQUEST better target its program to your classrooms and schools. The information is being collected and analyzed by the CyberQUEST evaluation team at the International Society for Technology in Education (ISTE) in collaboration with the project leadership. The survey asks for your name so that we can ask similar questions later in the project to see if your opinions have changed over time. However, no names or personally identifying information will be used in any reports.

Most questions are required in order to progress through the survey, but if you prefer to not respond to any item, simply mark any response and explain in the "notes" field that you wish that item to be left blank. Questions about the survey can be directed to Talbot Bielefeldt, Senior Research Associate, at talbot@iste.org Questions about participation in the project can be directed to the principal investigators [Dr. Katherine Hayden, at khayden@csusm.edu or Dr. Youwen Ouyang at ouyang@csusm.edu.

If you have any questions about your rights as a research participant, you may contact the CSUSM Institutional Review Board at 760.750.4029.
