* 1. Physician acknowledges the following:

The Policy and the associated documents relating to the Policy (Schedule(s), Attachment(s), etc.) supersede and replace entirely any and all prior versions of the Policy.
 
It is the responsibility of Physician to read and comply with the Policy. It is also the responsibility of Physician to question and discuss with their Divisional Managing Director, Human Resources or President, anything in the Policy Physician does not understand.
 
Physician understands the Corporation reserves the right in its sole discretion, to change, revoke, suspend, terminate or deviate from any Corporation policies and procedures, including the Policy, in whole or in part, at any time and with or without cause. Any material changes to the Policy will be communicated to all Physicians.

THE POLICY IS NOT A CONTRACT. PHYSICIAN UNDERSTANDS THE PROVISIONS OF THE POLICY, AND ALL OTHER CORPORATION POLICIES, PROCEDURES AND STATEMENTS, WHETHER ORAL OR WRITTEN, ARE NOT INTENDED TO CREATE AND DO NOT CREATE, AN EXPRESS OR IMPLIED CONTRACT OF EMPLOYMENT, OR ANY OTHER CONTRACTUAL RIGHTS, OBLIGATIONS OR RELATIONSHIPS OF ANY KIND, BETWEEN PHYSICIAN AND CORPORATION. PHYSICIAN ALSO UNDERSTANDS THAT NO IMPLIED CONTRACT CONCERNING ANY TERM OR CONDITION OF PHYSICIAN’S EMPLOYMENT CAN BE ESTABLISHED BY ANY CONDUCT OR PRACTICE OF THE CORPORATION.

Physician also acknowledges they have either carefully read the paragraph above or have had it read to them.

Physician understands the interpretation, application and administration of all Corporation policies and procedures, including those in the Policy, rest exclusively with the Corporation.

Physician has read, understands and agrees to abide by the Corporation’s Physician Code of Conduct Policy.
