COACH kick off

The Camden Coalition of Healthcare Providers is conducting a quality improvement process to understand the experiences of supervisors of healthcare workers implementing the COACH framework. Information collected through this process will help us understand challenges and successes associated with applying the COACH framework to supervision so we can continue to build on and improve the COACH framework. 

In this survey we are asking questions about your professional background and experiences so we can gain insight into how to refine training for professionals with different backgrounds. We are also collecting demographic information to better understand and describe the diversity of staff who have participated in COACH training.

The survey will take no more than 8 minutes to complete. Your responses will have no bearing whatsoever on your position with your employer; the data are being collected for program improvement purposes only and will be analyzed by Camden Coalition staff. Results will only ever be reported in aggregate form; no individualized or personal information will be shared.

Your participation is voluntary. Whether you do or do not agree to complete the survey will have no impact on your position with your employer.
