Dialoguing about Diversity

 

Informed Consent

The purpose of this research is to gain a better understanding of diversity in the Greater Cincinnati region. By clicking below and taking this survey you are consenting to have your data used in the results. The survey should take approximately 20 minutes. The benefit of participating in this study is that will provide researchers with a better understanding diverse interactions in the Greater Cincinnati area. You will be asked about your experiences interacting with different groups, about your feelings towards different groups and about your feelings about communication. There are no foreseeable risks for you from completing this survey. You will never be asked any questions in the survey that will compromise your identity, although you will be asked to provide demographic data at the beginning of the survey. Please understand this demographic data will not be released with any specific survey results and only in aggregate form. You will also be given the opportunity to enter your first name and either e-mail or phone number. This information will be kept separately from the previous data and will be used to enter you in a drawing to win one of four $50 gift certificates. You are not required to enter the drawing.

Your participation is strictly voluntary and you may stop participation at any time. Responses will be completely anonymous. The only people who will have full access to the data pool will be the researchers (Stephanie Klatzke, Jimmie Manning, Jacqueline McNally, Brooke Buckley, Kaitlyn Maxwell, and Igor Ristic). Your demographic information will not be linked to your responses in any internal or external reports. The data will be stored in the researcher’s locked office and will be kept for a minimum of 5 years.

By clicking below I understand that I consent to participate in this research project conducted by Stephanie Klatzke, Jimmie Manning, Jacqueline McNally, Brooke Buckley, Kaitlyn Maxwell, and Igor Ristic. I understand I will be taking a survey. My participation is voluntary, and I know that I am free to not participate in the study or to withdraw from the study at any time with no penalty. I will not be required to give my name, and my demographic data will only appear in aggregated form when this research is presented or published.
