Thank you for taking the time to complete the Neighborhood Church 2010 Child and Youth Census!
As the name implies, this survey has been created to gather information about the children living in your household. This form has spaces for up to 4 children/youth. Please keep in mind that the child you designate as "Child 1" in the first section will be "Child 1" throughout the census. Likewise for your other children.
As with the adult survey, the information that you provide will help the church communicate more effectively with you, it will provide the staff with information that may be helpful in the event of an emergency, and it may be used by boards and committees to create programs and or provide services.
All information that you provide will be stored in our church database which is a secure environment. Your child's contact information may be used in our Children's and Youth Ministries, including Church School and Youth Group rosters. All Neighborhood Church directories and rosters may be used solely for communication between and among the congregation and staff and may not be used for other purposes.
The other information you provide - such as interests or willingness to serve -- may be provided to church group or event organizers in the form of a list, roster, or report.
