Introduction: You have been asked to participate in this research study, which is being conducted by Leo Schlosnagle, Ph.D. at the Center for Excellence in Disabilities (CED) at West Virginia University (WVU) in collaboration with Amanda McBean, M.S., (of the Dept. of Psychology at WVU)
Purpose of this Research: The purpose of this research is to learn more about how parents of children sleep, as well as their health and ability to remember. The CED expects to enroll approximately 345 subjects.
Description of Procedures: This research involves the completion of an online survey and will take approximately 10-15 minutes for you to complete. You will be asked to fill out questionnaires regarding your child/children (if you have any), your sleep habits, your ability to remember, and your health. You will also be asked basic demographic information (such as your age, sex, and ethnicity). You do not have to answer all the questions, and can stop at any time.
Risks and Discomforts: There are no known or expected risks from participating in this research, except for the mild frustration associated with answering the survey questions. If you are uncomfortable answering any survey question, please skip that question.
Alternatives: You do not have to participate in this research. You may choose to not participate in this research now, or at any time while completing the surveys without penalty.
Benefits: You may not receive any direct benefit from this research. The knowledge gained from this research may eventually benefit others.
Financial Considerations: You will not incur any costs as a result of participating in this research.
Considerations for WVU students: WVU students’ grades will not be affected negatively, nor will their academic standing with WVU be affected as a result of participating in this research, or as a result of refusing to participate in this research.
Compensation: WVU students who participate in this research will receive course credit for their participation.
Confidentiality: None of the information you provide in this survey can be identified as originating from you. Any information about you that is obtained as a result of your participation in this research will be kept as confidential as legally possible. Your research records and test results may be subpoenaed by court order without your additional consent. No information from which you might be identified will be published without your consent. The Principal and Co-Investigator listed above are the only people who will have direct access to the raw data that you provide on these surveys.
Voluntary Participation: Participation in this research is voluntary. You are free to withdraw your consent to participate in this research at any time. Refusal to participate or withdrawal will not affect you in any way and will involve no penalty to you. In the event new information becomes available that may affect your willingness to participate in this research, this information will be given to you so that you can make an informed decision about whether or not to continue your participation.
