Health Program Questionnaire

Peg Connolly, health teacher and chair of the Health Policy Committee at Hopkins School and Rosemary Baggish, Director of Mental Health in Independent School Communities, have collaborated on this questionnaire about Health Programs in our schools. Members of the Hopkins School Health Policy Committee are examining their own health curricula and additional programming and are asking for your help with information gathering. Please take a few minutes to answer these survey questions about the health requirements and programs your school may have in place.
