This is the first part of a survey being conducted at the direction of the KHSAA Board of Control in compliance with KHSAA Bylaw 13. We remind schools that completing this survey is a mandatory membership requirement. This survey is to be completed by ALL KHSAA member schools, public, private and parochial. Based on selected answers, additional information will be requested on a subsequent survey.

{For example, Lexington Catholic HS is physically located in the Lafayette (Fayette County) District or Sacred Heart Academy is physically located in the Waggener (Jefferson County) District}.

* 3. What is the full, unsubsidized per capita cost of education for each student in the school district where the school is located per question #2 (non-public schools, this may require consultation with your local public school district to ascertain the accurate answer).

(This includes private and out-of-district students. This may be tuition collected by the school, tuition collected by the school district or tuition collected through a third-party agency. Please be diligent when researching the answer to this question as we have found that though many schools do not collect tuition from out of district students, their school district does and many public schools that in fact, do charge tuition to certain students, have previously answered this question "no". Any tuition collected would require the response to this question to be "yes").
