Thank you for your interest in serving as a member of the ASHHRA Board of Directors. Board charges are listed on www.ashhra.org--click on About ASHHRA and Governance. To be considered as a candidate, this form must be completed by April 5, 2011. Next, please send your resume and photo to ashhra@aha.org.

4. If selected for the ASHHRA Board of Directors, I am available to attend the September 9, 2011 breakfast and board meeting prior to the Annual Conference in Phoenix, Ariz.
