Choosing and Using Home Blood Glucose Monitors
1. Purpose of this Survey
Hello and thank you for volunteering to participate in this brief 29 question survey which should take less than 10 minutes to complete.
On Wednesday, March 17, 2010, I will be presenting "How Meters are Used at Home and How Consumers Choose Meters" at the FDA/CDRH* Public Meeting on Blood Glucose Meters. (*US Food and Drug Administration/Center for Devices and Radiological Health).
The purpose of this survey is to gather accurate information regarding YOUR PERSONAL EXPERIENCE with home BLOOD GLUCOSE METERS, and to summarize the results for the attendees at the meeting mentioned above. This survey is intended for residents of the United States.
Some of the questions allow for more than one answer to be checked. At the end of the survey there will be an optional text box in which you may add any additional information you believe is important to be shared during the FDA/CDRH meeting.
