Dear Parent,

I am conducting research to aid parents such as yourselves to gain an understanding of why they may or may not view their child’s futures as hopeful.This is a volunteer opportunity for you to offer your opinions about why you believe your child has been diagnosed with autism disorder and how you view your child’s future.
I am a graduate student who is undertaking this research at Alliant International University in San Francisco, California under the supervision of a faculty member and as part of the requirements for a doctoral degree in psychology. This research is presented as a survey aimed at parents of children with autism. Parents who are interested in understanding the impact that their views about their child’s diagnosis have on their child’s future are encouraged to participate. All information will be anonymous, thus, you will not be asked to provide ANY identifying information about yourself. If this seems like something that you would be interested in being a part of, please click on the link at the bottom of this page to complete the survey. This survey will take approximately 25 minutes. Your child’s other parent(s) may also choose to complete the survey though it must be done separately.
Please be sure not to give any identifying information such as your name, date of birth, social security number, or address. When you are finished with the survey, please click on the submit button on the final page. Please be informed that your participation is appreciated but it is entirely voluntary. If you would like to terminate participation, you may do so at any time. Once you submit your survey, you will not be able to withdraw from participation in this study. Any information submitted will be discarded after five years of the completion of this study. If you wish to receive a summary of the results of this study, please email me at rgrant@alliant.edu. No individual results are available because all of the data will be combined. The summary of the research results will be sent to you within 6 months of its completion, by email or registered mail after being reviewed by the Allient International University Institutional Review Board. You may receive a summary of results of this study whether you completed the survey or not. If you experience any emotional discomfort as a result of taking this survey please email me and I will provide the name of a licensed psychologist who may be able to assist you. You may choose to remain anonymous when emailing. Thank you for your assistance with this research project.
