1. Welcome to the Athlete Support and Skill Assessment Questionnaire

Thank you for your interest in this research project. Over the next couple of pages there are a series of statements which refer to the resources of your sport’s support system, and your life skills as an athlete. Please rate how much you disagree or agree with each statement at this time point.

There are no right or wrong answers, and all answers will be kept confidential. Only the researcher will see your personal responses. This is to ensure that you are comfortable about giving honest answers. The questionnaire will take between around 20 minutes to complete. Please do not dwell on questions and try answer every question – if you are not sure, go with your initial reaction.
