You are about to serve as a participant in a psychological experiment. The purpose of this research is to better understand how people understand food. The study is expected to take no more than 15 minutes to complete. During this time you will be judging different health bars and evaluating them. A detailed set of instructions follows on the next page.

Participation in this study is completely voluntary. You have the right to decline to participate and withdraw from the research now or at any time after the study has begun. If you decide not to continue please inform the research assistant. There are no negative consequences of declining or withdrawing. There are no other foreseeable factors that might cause you potential risk, discomfort, or adverse effects.

In what follows you may be asked to provide some personal information about yourself such as your sex, ethnic background or other sensitive information. This information will be kept strictly confidential. At no time will individual names be used when analyzing the data or publishing the results. Individual data will be identified only on the basis of a participant number.

There are several prospective benefits of this research. It will allow us to better understand the knowledge that people have over what they eat. A more complete description of the potential benefits of this investigation will be provided in the debrief form you will read after the study is completed.
