2012 Conference Registration

The 2012 OCADVSA Annual Conference will be held June 20 & 21at the Renaissance Hotel & Convention Center, 6808 S. 107th East Avenue in Tulsa.

Early registration will be at the Reception area of the Conference Meeting Space adjacent to the hotel from 3:30 - 5:30 p.m. on June 19. Check in time at the hotel is 3:00 p.m.

"Embracing Our Roots" Reception & Awards Banquet will be held June 19, beginning at 6:00 p.m. in Salon V in the Conference Meeting Space adjacent to the hotel.

OCADVSA member registration will cost $180 until June 1. Non-member registration will cost $205.

After close of business on June 1, all registration will be $205.

Walk-in registration (day of conference) will be $225.

NO REFUNDS WILL BE ISSUED FOR CANCELLATIONS AFTER JUNE 3.

4. Please list those attending. This registration includes the dinner reception on Tuesday evening. If splitting registrations, or registering someone using a scholarship, add that behind their name or please call Sharon Anderson at 405-524-0700.
5. Please state the name(s) of the person(s) attending the dinner on June 19, 2012 and if not attending the dinner, please put in the box below, "Not Attending Dinner".
6. We are striving to make our conference accessible to all participants. Do you need any equipment or particular arrangements (including dietary restrictions) in order to participate in the conference?
YOU WILL BE INVOICED FOR YOUR REGISTRATIONS. PAYMENT SHOULD BE MADE BEFORE THE CONFERENCE.

SPECIAL ROOM RATES AVAILABLE UNTIL JUNE 1 OR UNTIL BLOCK IS SOLD OUT. PROGRAMS RECEIVING SCHOLARSHIPS FOR ROOMS ARE RESPONSIBLE FOR MAKING YOUR OWN RESERVATIONS AND SUBMITTING RECEIPTS FOR REIMBURSMENT TO SHARON ANDERSON.
