1. Informed Consent
Thank you very much for your time.
This survey is designed with several purposes in mind.
First and foremost is an attempt to make the Springfield Regional Arts Council (SRAC) a better organization for its members, for the region and for the arts. The information gathered will be used by the SRAC to evaluate how it is doing and where it should go in the future to best meet its constituents needs and wants.
Additionally, the information will be used for a major graduate level research project in organizational communication.
What you can expect:
- The risks involved will be no greater than you would experience in every day activity.
- The survey should take between 10-15 minutes to complete.
- You can refuse to participate.
- You can refuse to answer any question that you do not feel comfortable answering.
- You can withdraw from the survey at any time prior to turning in the survey by simply canceling from this web-site.
- Once the data has been collected your results will be irretrievable and impossible to differentiate from the other respondents.
- Surveys are intended to be anonymous, but in any circumstance where individuals are identifiable by their responses, all identifying information will be removed before results are given to the SRAC or used for any academic paper or presentation.
- We will not be asking for names, but information about the key arts groups you associate with is vital for the research needs.
