This survey is being administered in support of our paper – “Leading for Process Safety Excellence” – to be presented at the 2012 4th Annual CCPS Latin America Process Safety Conference in July.

The survey contains questions designed to achieve the following:

1. Aggregate leadership profile information from the formal and informal process safety communities;
2. Identify process safety leadership development opportunities for the next generation of process safety professionals;
3. Improve the overall level of process safety leadership and process safety performance of the energy and chemicals industries.

Your input will be important to the industry as a whole.

Your opinions and responses are anonymous and nobody will attempt to determine your identity. In order to ensure anonymity, please do not write your name on any part of the survey.

Questions marked with a "(1)" include material from Peterson and Seligman's work on character strengths and virtues referenced below.

A codeword is provided on the last page of the survey. Please send that codeword in an e-mail to ashley.bourne@cognascents.com to ensure receipt of the final draft of the aforementioned paper.

Lastly, this survey is about process safety, not personal safety. In general, process safety focuses on workplace safety and personal safety focuses on worker safety.
