HS Community Needs Assessment

Hello. We would like families and individuals living in Westmoreland County to take this survey so that we may better understand the needs of county residents. This survey is designed to provide Westmoreland Community Action, Westmoreland County Head Start and Early Head Start programs with information to help plan and design programming for the county. This is an anonymous survey. By using the code below, we can make sure that we do not get more than one survey from the same person. Please enter all data requested.
