Dear St. Mary of Carmel School Family:
It is the ongoing task and responsibility of the school administration and the advisory board to effectively determine the ongoing needs of St. Mary of Carmel School. Thus, we are conducting a survey of the parents of our students. Your responses to this survey are crucial in providing necessary information to help formulate the school’s strategic plan for the next five years.
The purpose of this survey is to provide you an opportunity for input into your child’s (children’s) education. The information given by you will offer us important insight designed to both support and significantly contribute to a dialogue among all members of the school community as it relates to the school.
This is an online version of the survey sent home with your child. If you choose to take the online survey, you do not need to send in the paper version of the survey. Either way, the deadline for completion is Friday, May 28th.
Only one survey must be filled out per family. If 75% of school families return a completed survey to the office by May28th, the students will win an ice cream sundae party on the last day of school. For PK, K, and 8th grade, responses must be turned in by the last day of school for these classes (May 21st and 22nd) at a 75% response rate.
Important: If you take the survey online and choose to remain anonymous, please send a note to your child's teacher so that you will be counted for the ice cream sundae party contest. In the note, state the you have completed the survey online.
We value and respect the time you spend on responding to these questions. It is only through working together that we can accomplish our goal of providing the best possible Catholic education for the students of St. Mary of Carmel.
